Required Vaccination Policy: All pets are required to be up to date on the following vaccines: Canine - DA2PPL, Rabies, Bordetella, and Lepto. Feline - Rabies and FVRCP. If you answer "No" to the following question, you hereby approve Crossroads Animal Hospital to vaccinate your pet upon entering the boarding facility." Following with the same yes or no boxes and statements as seen in the picture followed by the previous veterinary hospital (if applicable). *

In case of illness or injury, I the undersigned, do hereby give my authorization and consent for the doctor to treat, prescribe to or operate upon my pet(s) while they are being boarded at the hospital. I authorize treatment for parasites and/or sedation for treatment or to control aggression, if needed. Crossroads Animal Hospital and our employees will use all responsible precautions against illness or injury but will not be held liable or responsible in any manner whatsoever, under any circumstances, on account of the care, treatment, or safe keeping of my pet(s) in any part of the facility (including Doggie Daycare), as it is thoroughly understood that I assume all risk.

I authorize the staff at this veterinary practice to release portions of my pet’s medical history and record, including personal recollections, radiographs, photographs, videotape images or other images to use with the following media sources: Crossroads Animal Hospital's website, Facebook page, TikTok or Instagram.

Please note: Crossroads Animal Hospital is not responsible for any personal items including collars, leashes, toys, or bedding that are lost or damaged by your pet during their stay.
